Chamblee Massage Ordinance

HOW TO APPLY FOR A NEW MASSAGE/SPA ESTABLISHMENT LICENSE
Step 1: Contact Licensing and Revenue Officer

Contact the licensing and revenue officer via email at alcoholbusiness@chamblee.com to verify that any vacancies for a massage / spa establishment exists before signing any legal documents.  If a vacancy exists, please proceed to Step 2.  If no vacancies exist, no applications for a massage / spa will be accepted.

Step 2: Apply for Occupational Tax Certificate

Begin the process of applying for an Occupational Tax Certificate by visiting the Online Business License Portal and creating an account for the Occupational Tax Certificate.


Step 3:  Apply Online

All applicants for new Massage / Spa Establishment Licenses must create an account for a Massage / Spa Establishment License and submit their completed application and supporting documents online at Massage / Spa Establishment Application Portal


 Step 4:  Fingerprinting + Background Check + Massage Work Permits 

All listed applicants and employees that do hold a current, valid massage therapist license issued by the state of Georgia are not required to obtain a work permit or submit the fingerprint acknowledgement form.

All listed applicants and employees that do not hold a current, valid massage therapist license issued by the state must complete the Fingerprint Acknowledgement Form and submit the form in person at Chamblee City Hall or via email to alcoholbusiness@chambleega.gov for authorization to proceed to the Police Department for Fingerprinting Services to obtain a Massage Work Permit. (See Information below)

Employee Work Permits are Required

Employee means any person who performs any service on the premises of a massage establishment or spa establishment, on a full-time, part-time, or contract basis, regardless of whether the person is denominated an employee, independent contractor, agent, apprentice, trainee, or otherwise. All employees performing services on premises are required to have state issued Massage Therapist License or a Chamblee Work Permit.  Please schedule an appointment with the City of Chamblee’s Police Department Records Section by calling 770-986-5005.  Appointments are available Tuesday, Wednesday, and Thursday between the hours of 8:00 AM and 4:00 PM. 

Massage Work Permits are $150.00 per person.  The charge is broken down into two payments.  One payment is for $100.00 for the application fee, photo, and permit card paid to the City of Chamblee via cash, credit card, or money order.  The second payment for $50.00 will be paid to GAPS via credit or debit card for the fingerprinting process.  The Massage Work Permit will be available within two business days after fingerprints are submitted.


Step 5: Onsite Inspection Performed by the Chamblee Police Department

Once you have submitted your complete application, completed the background check and fingerprinting process, and the Massage Work Permits have been obtained, the Revenue and Licensing Officer will forward the complete application packet to the Police Department to perform the onsite inspection.  Once you pass the onsite inspection, your application will move to Step 6.


Step 6: Pay the City of Chamblee Licensing Fees

Once you have passed the onsite inspection, applicants must submit the license fees directly to the City of Chamblee via Massage / Spa Establishment Application Portal.  Credit Cards and E-Checks will be processed by Stripe.  You will receive your local license via email within two business days of receipt of payment.
